(1A) Application Form (download and print). Complete and sign. Return to Summer Sessions. Mandatory for participation.
(2A) $200 Deposit. Deposit is non-refundable. Check should be made out to "U.C. Regents." Deliver payment directly to Summer Sessions. Mandatory for participation.
(3A) Statement of Purpose. Write a one page essay answering the following questions. Return to Summer Sessions. Mandatory for participation.
Why do you wish to travel to Querétaro, Mexico and what do you expect to learn from this program? How do you plan to integrate yourself into the Mexican family structure, society during your visit as an ambassador from your university?
(4A) Student Agreement (download and print). Read and sign. Return both pages to Summer Sessions. Mandatory for participation.
(5A) Waiver of Liability (download and print). Read and sign. A parental/guardian signature is needed on the Waiver ONLY IF you are UNDER the age of 18. You are required to sign the Waiver regardless of your age. Return to Summer Sessions. Mandatory for participation.
(6A) Housing Questionnaire (download and print). Complete and return to Summer Sessions.

(1B) Confidential Health History and Travel Clearance (download and print both forms). Fill out the Confidential Health History form first and bring it with you to your travel clearance appointment at Student Health. Since the travel clearance can be a lengthy process, do NOT wait to make an appointment at Student Health. Return both forms to Summer Sessions, plus one photocopy of each signed form. Mandatory for participation.
(2B) Registration Form (download and print). Complete and sign. Return to Summer Sessions.
(3B) Legal Documents for travel. Return photocopies to Summer Sessions (bring original documents with you to Mexico):
(4B) International Student ID Card. Purchase International Student ID card at STA Travel. Return two photocopies of International Student ID to Summer Sessions (bring original card with you to Mexico).
(5B) Travel Itinerary . Email or drop off one copy of your travel itinerary to Summer Sessions. Travel itinerary should include all arrival and return flight dates, times, airlines, and flight numbers.

Handle #1C-2C according to each item's instructions, pay special attention to different deadlines and recipients.
(1C) Family/Student Agreement/ English / Spanish. Give to your host family to sign at the start of the program and return to the Director.
(2C) BARC Refund Direct Deposit Form (download and print) Optional but strongly recommended . If you will be receiving financial aid, which will be applied to your BARC account, please download and fill out the Direct Deposit form for your BARC refund check. Turn in Direct Deposit form at least one month before the program start date.
