Forms and Documents
DIRECTIONS : Complete and return all forms and documents no later than the stated deadline directly to the Travel-Study drop-box in the Office of Summer Sessions (SAASB 2214). All documents listed in Sections A and B are mandatory for participation in the Turkey Travel-Study Program.
SECTION A— Deadline for priority consideration June 1, 2008.
- (A1) Travel-Study Application Form. ( download and print ). Complete and sign. Program Eligibility : UC students, good academic standing and 18 years of age or older.
- (A3) $200 Deposit. Deposit is non-refundable. The check is to be made payable to "UC Regents" and attached to the application form. Applications will not be accepted without deposit.
- (A4) Statement of Purpose. In a typed essay of no more than 250 words, explain why you want to attend the Turkey Travel-Study Program.
- (A5) Student Agreement. (download and print) Read and sign. Make a copy for your files. Return both pages of the original signed document to Summer Sessions.
- (A6) Waiver of Liability. ( download and print ) Read and sign. (Note: Parental signature in NOT needed for those over 18 years of age.) Make a copy for your files and return the original signed document to Summer Sessions.
SECTION B— Deadline August 1, 2008.
- (B1) Confidential Health History (download and print). Fill out the Confidential Health History form and bring it with you to your travel clearance appointment at Student Health or with your own physician.
- (B2) Travel Clearance (download and print). This form must be completed by a physician during a check up. Bring the Confidential Health History form with you to your travel clearance appointment. Since the travel clearance can be a lengthy process, DO NOT WAIT to make an appointment at Student Health or with your doctor. Return both forms to Summer Sessions.
- (B3) Passport. Turn in two photocopies of the signature and photo pages of your passport to Summer Sessions (bring original document with you to Greece) .
- (B4) Student Off-Campus Travel Accident Insurance. Students must register by going to www.uctravel.org and filling out the "Business Traveler Insurance" form.
- (B5) Travel Itinerary. Email or drop off one copy of your travel itinerary to Summer Sessions. Travel itinerary should include all arrival and return flight dates, times, airlines, and flight numbers.
SECTION C—Additional optional form(s)
- (C1) BARC Refund Direct Deposit Form. (download and print) Optional but strongly recommended. If you will be receiving financial aid which will be applied to your BARC account, please download and fill out the Direct Deposit form for your BARC refund check. Complete and submit Deposit form at least one month before the program start date.
- (C2) International Student ID Card. Optional. You can purchase this card at STA
Travel to save money on travel booked through them. Bring original card
with you to Turkey.
