TO APPLY FOR THE GREECE PROGRAM, YOU MUST CONTACT THE INSTRUCTOR TO ARRANGE AN INTERVIEW!!!
SECTION A — Deadline May 1, 2008

Complete and turn in directly to the Summer Sessions Travel Study drop-box in 2214 SAASB.
- Application Form (download and print). Complete and return to Summer Sessions. Program Eligibility: UC students, good academic standing and 18 years of age or older. Students who apply early will have first choice on where they prefer to study-Paros or Ithaca.
- $200 Deposit. Deposit is non-refundable. Check should be made out to "U.C. Regents." Deliver payment directly to Summer Sessions. Mandatory for participation.
- Statement of Purpose. In a typed essay of no more than 250 words, explain why you want to attend the Greece travel study program, why you have chosen Crete or Paros, and what strengths you will bring to the program. Staple your essay to the back of the application form and return to Summer Sessions.
- Waiver of Liability (download and print). Read and sign. A parental/guardian signature is needed on the Waiver ONLY IF you are UNDER the age of 18. You are required to sign the Waiver regardless of your age. Return to Summer Sessions. Mandatory for participation.
- Housing Questionnaire (download and print). Complete and return to Summer Sessions. Please include a recent photo.
SECTION B — Deadline May 15, 2008

Complete and turn in directly to the Summer Sessions Travel Study drop-box.
- Confidential Health History (download and print). Fill out the Confidential Health History form and bring it with you to your travel clearance appointment at Student Health or with your own physician.
- Travel Clearance (download and print). This form must be completed by a physician during a check up. Bring the Confidential Health History form with you to your travel clearance appointment. Since the travel clearance can be a lengthy process, do NOT wait to make an appointment at Student Health or with your doctor. Return both forms to Summer Sessions.
- Registration Form (download and print). Complete and sign. Return to Summer Sessions.
- Student Off-Campus Travel Accident Insurance. Students must register by going to www.uctravel.org and filling out the "Business Traveler Insurance" form. Click here for more information.
- Travel Itinerary. Email or drop off one copy of your travel itinerary to Summer Sessions. Travel itinerary should include all arrival and return flight dates, times, airlines, and flight numbers.
SECTION C — Additional documents

- BARC Refund Direct Deposit Form (download and print). Optional but strongly recommended. If you will be receiving financial aid, which will be applied to your BARC account, please download and fill out the Direct Deposit form for your BARC refund check. Turn in to the BARC Office at least one month before the program start date.
- International Student ID Card (Optional) You can purchase this card at STA
Travel to save money on travel booked through them. Bring original card
with you to Greece.
